Bug Fixes

This corrects an issue with the MIPS Promoting Interoperability(PI) showing 0 for the e-prescribing section.
Quality Measure # 128 Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan were not populating on the quality measures report.
For Measure 226 Criteria #1 - Removed the 8P modifier from Claims and Registry.
