Patient Lists

Patient Lists are a tool in TRAKnet that allow for a list of patients per certain criteria to be quickly, easily and effectively generated to help better patient care.

Patient lists are lists of patients based on a determined set of criteria. The most common purpose of this feature is for Meaningful Use, as detailed below, in the Meaningful Use portion of this page. Patient lists can be exported, or can be used to access a patient's chart, generate patient reminders, or view other pertinent information regarding a patient's medical data. Patient lists can be based on a variety of criteria as generic or specific as the practice sees fit.

Accessing Patient Lists

To access the Patient Lists tool in TRAKnet, click on the Reports dropdown menu in the upper left-hand portion of the main screen, then select Patient Lists. The Clinical Decision Rules and Patient Lists screen will then open.

This screen has two tabs:

  • Rules and Lists — This tab is where Clinical Decision Rules and Patient Lists are created.
  • Reports — This tab is where Clinical Decision Support Rules, Patient Lists and Reminder reports are displayed.

Creating Patient Lists

To create a new patient list, from the Rules and Lists tab click on the Patient Lists folder, then click Add. This will open the Add Patient Lists window.

This window is broken down into two sections:

  • Name — The name of the list you are generating. This should be descriptive enough to indicate the purpose of the patient list.
  • Data and Criteria — The criteria with which the list is filtered. Adding items to this filters out certain patients and is the true purpose of the patient list tool.

Under Criteria, there are several potential categories:

  • Patient — Patient specific demographic information such as age, gender, or ethnicity.
  • Problem — Diagnoses, used to generate a list of patients who are diagnosed with a specific problem, such as a list of all diabetic patients.
  • Medication — Used to generate a list of all patients with a history of a specific medication.
  • Medication Allergy — Used to generate a list of all patients with a history of a specific medication allergy.
  • Laboratory — Used to generate a list of all patients with a history of a certain lab.
  • Vital Sign — Used to generate a list of patients based on height, weight or blood pressure.

To add a new criteria, click the Add button, select the type of criteria from the above list, and follow the prompts to determine what specific data in that category you are looking to use.
