This page will explain the Promoting Interoperability measures for MIPS, and how to perform those measures in the TRAKnet application.

What is Promoting Interoperability?

Promoting Interoperability, or PI, is one of four categories which contribute to your overall MIPS composite score. PI replaces Meaningful use, using many of the same measures but dividing them up into required base measures, performance measures, and optional extra credit measures.

To receive credit for the base measure's providers must submit for all five base measures available in the TRAKnet application. To improve on the base score the provider can complete the performance measures, these will improve the overall score based on their completion. Lastly, the provider can complete the bonus measure to improve their overall score.

The Promoting Interoperability Report

The PI report can be found in TRAKnet under reports, in the quality payment program section. The report can be filtered by provider, location or date range and can be viewed in a summary version or detailed version. The summary version provides a summary of all measures including the numerator, denominator, percentage of completion and the required percentage for the measure. The detailed version shows a breakdown for each individual measure which includes the number, denominator, percentage completed and a list of patients who qualify for this measure.

TRAKnet and MIPS/MACRA – Promoting Interoperability

TRAKnet Version 3.1 is 2015 certified and is eligible for use for the completion of MIPS/MACRA 2018.

Attestation

After completing the required steps for the reporting period, you must attest your completion of the Promoting Interoperability measures. The CMS Registration and Attestation site CMS Registration and Attestation Site can provide relevant links and attestation information to aid in the attestation process.

Electronic Prescribing

CMS Documentation

Measure: At least one permissible prescription written by the MIPS eligible clinician is queried for a drug formulary and transmitted electronically using CEHRT.
Required for Base Score (50%): Yes
Percentage of Performance Score: 0
Eligible for Bonus Score: No

Provide Patient Access

CMS Documentation

Measure: At least one patient seen by the MIPS eligible clinician during the performance period is provided timely access to view online, download, and transmit to a third party their health information subject to the MIPS eligible clinician’s discretion to withhold certain information.
Required for Base Score (50%): Yes
Percentage of Performance Score: Up to 10%

Request/Accept Summary of Care

Measure: For at least one transition of care or referral received or patient encounter in which the MIPS eligible clinician has never before encountered the patient, the MIPS eligible Clinician receives or retrieves and incorporates into the patient's record an electronic summary of care document. Security Risk Analysis

Measure: Conduct or review a security risk analysis in accordance with the requirements in 45 CFR 164.308(a)(1), including addressing the security (to include encryption) of ePHI data created or maintained by certified EHR technology in accordance with requirements in 45 CFR164.312(a)(2)(iv) and 45 CFR 164.306(d)(3), and implement security updates as necessary and correct identified security deficiencies as part of the MIPS eligible clinician's risk management process.

Send Summary of Care

CMS Documentation

Measure: The MIPS eligible clinician that transitions or refers their patient to another setting of care or health care clinician (1) uses CEHRT to create a summary of care record; and (2) electronically transmits such summary to a receiving health care clinician for at least one transition of care or referral.

Performance Score Measures


Clinical Information Reconciliation

CMS Documentation

Measure: For at least one transition of care or referral received or patient encounter in which the MIPS eligible clinician has never before encountered the patient, the MIPS eligible clinician performs a clinical information reconciliation. The MIPS eligible clinician must implement clinical information reconciliation for the following three clinical information sets: (1) Medication. Review of the patient’s medication, including the name, dosage, frequency, and route of each medication. (2) Medication allergy. Review of the patient’s known medication allergies. (3) Current Problem list. Review of the patient’s current and active diagnoses.

Immunization Registry Reporting

Measure: The MIPS eligible clinician is in active engagement with a public health agency to submit immunization data.
Required for Base Score (50%): No
Percentage of Performance Score: 0 or 10%

Patient Generated Health Data

CMS Documentation

Measure: Patient-generated health data or data from a non-clinical setting is incorporated into the certified electronic health record technology (CEHRT) for at least one unique patient seen by the MIPS eligible clinician during the performance period.
Required for Base Score (50%): No
Percentage of Performance Score: Up to 10%

Patient Specific Education

Measure: The MIPS eligible clinician must use clinically relevant information from certified electronic health record technology (CEHRT) to identify patient-specific educational resources and provide access to those materials to at least one unique patient seen by the MIPS eligible clinician.

Secure Messaging

CMS Documentation

Measure: For at least one unique patient seen by the MIPS eligible clinician during the performance period, a secure message was sent using the electronic messaging function of certified electronic health record technology (CEHRT) to the patient (or the patient-authorized representative), or in response to a secure message sent by the patient (or the patient-authorized representative).
Required for Base Score (50%): No
Percentage of Performance Score: Up to 10%
Eligible for Bonus Score: No

View, Download and Transmit

CMS Documentation

Measure: During the performance period, at least one unique patient (or patient authorized representatives) seen by the MIPS eligible clinician actively engages with the EHR made accessible by the MIPS eligible clinician by either—(1) viewing, downloading or transmitting to a third party their health information; or (2) accessing their health information through the use of an Application Programming Interface (API) that can be used by applications chosen by the patient and configured to the API in the MIPS eligible clinician’s certified electronic health record technology ( CEHRT); or (3) a combination of (1) and (2)
